Quanta point-of-care assessment of platelet function in cardiac surgery: a comparative study of sonorheometry, rotational thromboelastometry ROTEM, platelet function analyser PFA-200 and clinical assessment of bleeding.

Bleeding is a major problem during cardiac surgery and platelet dysfunction contributes to coagulopathy. Early recognition of platelet-dependent bleeding can facilitate targeted platelet transfusion, however current point-of-care testing (POC) methods lack strong evidence and cuff-off values to prompt intervention.

The Quanta System is a newer device which measures clot elasticity. This may be a better measure of platelet function after cardiopulmonary bypass compared to our standard POC tests (ROTEM and PFA-200). It may also correlate better with surgeons’ evaluation of intraoperative bleeding. This prospective observational pilot study will compare the results of the Quantra System with ROTEM, PFA-200, and the operating surgeons’ clinical assessment of bleeding after cardiopulmonary bypass, in patients undergoing cardiac surgery with a higher risk of bleeding.
